For approval, the applicant will attest that the requirements are met. As of Oct 1, 2022 the policy is that the RAEs will accept attestations, but not the entire set of documents.
To begin, email the RAEs and ask for their "supervision endorsement packet" or "supervision attestation form"

The supervisee’s mandatory disclosure statement must clearly state that they are under supervision and by whom.
HPCF/RAEs require "regular evaluation of the supervisee’s progress with a rubric that is tied to the responsibilities assigned."

A "policy describing how providers credential their practitioners, requiring monthly checks of federal exclusion databases (OIG and SAM) of the unlicensed providers" is required.
